Cigna is a health insurer whose pharmacy benefit business earns more than the insurance itself.
The insurance half serves employers: corporate plans for staff. That is a more predictable segment than government programmes, because the insured population is younger and healthier and terms are revised annually.
Most profit comes from Evernorth, the pharmacy benefit arm: it negotiates rebates with manufacturers on behalf of health plans and runs pharmacy fulfilment. Rebate mechanics are opaque and a recurring subject of investigation, but they are what provide scale and margin.
Risks: political pressure on the middleman's role in drug pricing, rising utilisation, and competition from vertically integrated rivals. Cigna pays a dividend and buys back stock actively.
